Hiring for an RN

3 days/week-MWF + on-call

Wage range $35-$48/hour

(negotiable based on experience)

The Registered Nurse (RN) provides clinical leadership and oversight for 12 residents residing at South View Living Center in Gibbon, MN. The RN is responsible for resident assessments, care planning, medication and treatment oversight, staff education, regulatory compliance, and collaboration with residents, families, physicians, and the interdisciplinary team. This position promotes person-centered care while ensuring compliance with Minnesota Assisted Living licensure requirements and supporting a safe, compassionate environment for older adults, including those living with Alzheimer's disease and other dementias.

Clinical Care: The Registered Nurse completes preadmission, initial, and ongoing resident nursing assessments in accordance with applicable regulations and facility policies and develops, implements, evaluates, and updates individualized resident service plans. The RN monitors residents for changes in physical, cognitive, emotional, and functional condition and ensures appropriate follow-up and communication with physicians, nurse practitioners, families, and other healthcare professionals. Responsibilities include coordinating physician orders, laboratory services, pharmacy services, therapy, hospice, home health, and other outside healthcare services. The RN ensures timely, accurate, and complete documentation in the electronic health record and provides clinical oversight of medication management, medication administration, treatments, and other nursing services. The RN also reviews clinical records, medication administration records, and treatment documentation for accuracy and compliance.

Memory Care: The RN promotes compassionate, person-centered dementia care practices and provides staff guidance regarding behavioral expressions, dementia-related changes, and individualized approaches to care. The RN encourages the use of appropriate non-pharmacological interventions and assists staff in identifying strategies that promote resident comfort, dignity, safety, and quality of life. The RN provides education and support to families regarding dementia progression, changing care needs, and care planning and participates in resident care conferences and interdisciplinary meetings. The RN advocates for each resident's dignity, independence, preferences, and quality of life throughout the progression of dementia.

Regulatory Compliance: The RN maintains compliance with Minnesota Statutes Chapter 144G, applicable Minnesota Department of Health requirements, the Minnesota Nurse Practice Act, and facility policies and procedures. The RN assists with regulatory survey preparation and participation and assists in developing and implementing corrective action plans when necessary. Responsibilities include maintaining accurate, complete, and confidential resident clinical records and participating in the development, implementation, and review of clinical policies and procedures. The RN supports ongoing compliance with resident rights, infection prevention, medication management, emergency preparedness, and other applicable regulatory requirements.

Staff Education and Development: The RN provides clinical orientation and ongoing education for caregivers and other clinical staff and conducts medication administration, delegated nursing, and competency-based training as appropriate. The RN reinforces infection prevention and control practices and provides education related to resident safety, fall prevention, dementia care, emergency preparedness, and recognizing and reporting changes in resident condition. The RN serves as a knowledgeable and approachable clinical resource and mentor for direct-care staff and promotes continued professional development throughout the care team.

Resident and Family Relations: The RN develops positive, professional, and trusting relationships with residents and their families and communicates proactively regarding changes in condition, care needs, and service planning. The RN responds promptly and professionally to resident and family questions or concerns and participates in resident and family care conferences as appropriate. The RN coordinates hospice, home health, therapy, pharmacy, medical providers, and other ancillary services and promotes a collaborative approach to care that keeps residents and families informed and involved in care decisions.

Qualifications: The Registered Nurse must hold a current, unrestricted Registered Nurse (RN) license in the State of Minnesota and have at least one year of nursing experience; experience in geriatrics, assisted living, memory care, long-term care, or home health is preferred. Current CPR certification is required. The RN must demonstrate strong nursing assessment and clinical judgment skills, excellent communication, leadership, and organizational abilities, and knowledge of dementia care principles and best practices. The RN must prioritize responsibilities and work independently while collaborating effectively with an interdisciplinary team, and must maintain confidentiality and professionalism in all resident, family, and staff interactions. Previous Assisted Living or Memory Care experience, experience with electronic health records such as ResiDex or similar systems, knowledge of Minnesota Assisted Living regulations, and experience with nursing delegation, medication management, staff education, and clinical oversight are preferred.

Physical Requirements: The RN must be able to perform the physical functions necessary to safely fulfill the responsibilities of the position, with or without reasonable accommodation. This includes the ability to stand and walk for extended periods and perform frequent bending, reaching, stooping, and kneeling. The RN must be able to assist residents with mobility and transfers as necessary, lift or assist with up to 50 pounds when appropriate and with assistance or equipment as required, and respond promptly and effectively during resident emergencies.
